If you are interested in requesting paperwork for LPC and/or QMHP C or a request for verification form must be filled out and emailed to the department administrative assistant at amwiggins@nsu.edu.
Please allow 10 business days from the form being sent in for processing for the paperwork to be sent electronically to the licensure board.
