SAP Undergraduate Academic Plan (pdf)
A required form for undergraduate students submitting a SAP appeal. This plan details the steps the student will take to meet SAP requirements and continue receiving financial aid.
SAP Graduate Academic Plan (pdf)
A required form for graduate students submitting a SAP appeal. Outlines a personalized academic plan to help the student meet SAP standards and maintain financial aid eligibility.
SAP Degree Audit Form (pdf)
Used to track a student’s academic progress toward degree completion. This form helps confirm that the student can complete their program within the allowed timeframe for financial aid eligibility.
SAP Appeal Form (pdf)
This form is used by students who have not met the SAP standards and wish to appeal for the reinstatement of their financial aid. It requires a personal explanation of the circumstances that affected academic performance, supporting documentation, and a plan for academic improvement.
Spartan Family Plan Application (pdf)
The Spartan Family Plan provides an incentive to students with immediate family members enrolled at the University who are seeking an affordable, high-quality education. If awarded, an adjustment to the final balance of a student’s cost of attendance will be made. Award amounts will be determined based upon need and after an administrative review of the application and students’ records.
TEACH Grant Application (pdf)
Teacher Education Assistance for College and Higher Education Grant Program provids grants to students who are completing or plan to complete course work needed to begin a career in teaching.
Transfer Grant Application (pdf)
The NSU Transfer Student Grant is awarded on an availability basis to registered transfer students based on demonstrated financial need and merit.

These forms may be downloaded, printed, completed and returned to the Financial Aid Office or uploaded in PDF or jpeg format and attached to an email. Photographed images (taken from a camera or cell phone) are not acceptable. All documentation must be signed (typed scripted fonts and Docu-sign are not acceptable) and dated by the student and/or parent.
