New Incident Reporting System to Promote Safety and Inclusion

Norfolk State University (NSU) is committed to ensuring an inclusive and welcoming environment for students, staff, and faculty, and strives to provide an accepting place for all to work and learn.  NSU asks for institution-wide cooperation in monitoring the campus climate by reporting all incidents and concerns and recording all requests. 

Beginning April 1, 2022, an incident reporting system (Guardian by Campus Kaizen) for incidents of harassment and discrimination and accommodation requests, is now available. With the leadership of the Office of Information Technology, this system can be utilized by internal and external community members. The system implementation is the result of a major collaboration with the NSU Police Department, Residential Life and Housing, the Dean of Students Office, and the Office of Institutional Equity.

The system will be updated periodically throughout the year. NSU appreciates your support in trying to eliminate the use of paper while addressing incidents across the university, in a timely manner.

The following concerns, incidents, or requests regarding NSU students, staff, or faculty should be reported using this system;

  • Accommodation Request
  • Bias Reporting
  • Title IX Sexual Harassment
  • Discrimination/Harassment based on the following:
    • Age 40 and Over
    • Color
    • Disability
    • Genetic Information
    • National Origin
    • Pregnancy
    • Race
    • Sex (including gender identity and sexual Orientation)
    • Veterans

Any emergency situation should ALWAYS be reported to the NSU Police Department, first. Emergencies can be reported by calling the NSU Police Department at 757-823-9000.
