Information regarding requests for exemption from COVID-19 Vaccination

COVID-19 is a highly contagious respiratory virus that affects people of all ages. This virus can cause long-term medical problems and death regardless of age. This virus spreads through respiratory secretions related to speaking, singing, yelling, coughing, and sneezing. Infected individuals can spread the virus to others. Prevention strategies include wearing a mask and physically distancing when around others.

The COVID-19 vaccines are very safe and highly effective at preventing death and hospitalization. When large numbers of people within a population are immunized, viral spread will be significantly limited. Each individual within a community can contribute to this protective approach.

Side effects related to vaccination primarily include pain at the injection site, headache, fever and chills, or body aches that last for about 24 hours.

Exemptions for Students

Religious Exemption – The Religious Exemption Form must be downloaded and notarized in order to be compliant with the Commonwealth of Virginia’s policy. Please complete the form, have it notarized, and then send a copy of the completed form to COVID@nsu.edu or you may provide a paper copy to The Vice President of Student Affairs Office in The Student Services Center, Suite 301. 

Medical Exemption - For a medical exemption, you must have documentary proof on official letterhead from a healthcare provider or local health department that one or more immunizations may be detrimental to the student's health. Please send your documentation securely to the Spartan Health Center by Fax 757-823-2234 or email spartanhealth@nsu.edu.

Exemptions for Employees

Employee medical and religious exemptions can be applied for through the Office of Institutional Equity.

Religious Exemption – The Religious Exemption Form must be downloaded and notarized in order to be compliant with the Commonwealth of Virginia’s policy. Please complete the form, have it notarized, and then send a copy of the completed form to the Office of Institutional Equity in person or email to TOIE@nsu.edu.
